How do I arrange a locum?
Once you have 
confirmed your eligibility for a locum, you will need to lodge a “Request for Locum Assistance” form.
This can be done in two ways:
1. Online by completing an Online request for Locum form.
2. Download and complete an Request for Locum form. This form can be faxed or posted to the Locum Service.
When completing request forms, it is essential to provide as much information/detail as possible to ensure all aspects of the request are recorded: eg consulting session days and times, after hours requirements, accommodation options.
Each request is allocated an acknowledgement number that is forwarded to the rural GP. This number needs to be quoted to make any alterations to the initial request details.
